·        CKC Registration Papers & Microchip

·        Immunizations and De-wormings up to date

·        Veterinary health certificate

·        5 yr genetic health guarantee, *not including hypoglycemia*

·        Puppy pack with puppy care information and sample of current food

·        Lifetime return policy and Breeder support

·        $100 refund incentive to have puppy spayed/neutered by the age of 6-8 months

I, the breeder:

·        Do not guarantee the adult weight or color of the puppy.

·        Will replace the above puppy within the first 5 yrs of his/her life, if he/she is found to have a life threatening genetic health condition. Licensed veterinarian documentation is required to be eligible for replacement with the next available puppy. Hypoglycemia is not covered under this guarantee.

·        Will take the above puppy back at any time in his/her life, if the new owners are unable to keep him/her, for any reason. No money will refunded and every effort will be made to find the above dog a suitable home.

·        Will refund the buyer $100 when proof from the vet is provided, that the above puppy has been spayed/neutered by the age of 6-8months.

·        Will provide ongoing breeder support to the buyer for the life of the puppy, in the way of information & advice.

·        Agree that this contract is binding at the time/date of signing.

I, the buyer:

·        Agree to provide proper love, care and exercise to the above puppy for his/her entire life. A healthy diet and socialization will be provided to ensure a balanced life for the above puppy.

·        Agree to have the above puppy examined by a licensed veterinarian within 3 business days of taking the puppy home, failure to do so will void this contract. Keep records please.

·        Agree to educate myself on hypoglycemia & know what to do in an emergency situation. I understand that this is not covered under this agreement.

·        Agree to keep the above puppy’s immunizations and dewormings up to date as the dog ages, and seek medical treatment when needed for any illness.

·        Agree to keep care of the above puppy’s teeth for his/her entire life, and to have them regularly examined by my vet.

·        To notify the breeder the same day if any life threatening genetic condition is discovered in the above puppy. If the above puppy is euthanized for this reason, an autopsy is required and a report will be provide to the breeder, before any replacement will be provided. This is at the cost of the buyer.

·        Agree that I am responsible for all costs incurred by the above puppy for its entire life, from the time of adoption.

·        Agree to notify the breeder if I can not keep or decide not to keep the above puppy, for any reason. I agree never to turn the above puppy over to a shelter or puppy mill. I understand that no money will be returned if I return the puppy to the breeder, and all effort will be made to find a healthy home for him/her.

·        Agree that this contract is binding at the time/date of signing.
